1. PURPOSE

• I'll provide, and you'll pay for, dementia care coaching and support services for individuals supporting a family member through a care transition (such as a move to a new care setting, a change in caregivers, or another significant shift in care), in accordance with the terms below.

2. WHAT'S INCLUDED: Care Transition Support Package

• One 60-minute + one 30-minute virtual coaching session via Zoom over 2 weeks.
• Recordings of both sessions.
• A comprehensive self-care plan covering: stress and guilt processing, self-care routines, boundary-setting, time management, support systems, and emotional resilience through the transition.
• A comprehensive transition and adjustment plan covering: evaluating and choosing the right care option, transition-day logistics, communicating with new care providers, advocacy and visitation strategies, and supporting your family member's comfort, routines, and sense of identity as they adjust to the change.
• Unlimited email access to me for 4 weeks following your second session.
• Lifetime 25% discount on all future programs and offerings.
• Lifetime access to the Resource Vault in the AttractWell App.

3. SCHEDULING & CANCELLATION

• Sessions will be scheduled at mutually agreeable times.
• I'll make every effort to schedule your first session within 3 days of your completing the pre-coaching homework.
• Either of us needs to give at least 24 hours' notice to cancel or reschedule a session.
• Sessions cancelled with less than 24 hours' notice are non-refundable.
• If I need to cancel and can't reschedule within 5 days, you're entitled to a refund for that session.

4. PAYMENT

• Total fee: $299, paid in full at time of booking.
• Coaching begins once payment is received.
• Additional sessions are available at $120/hour (in 30-minute intervals).

5. REFUND POLICY

• Payments are non-refundable. This isn't meant to feel harsh. It protects the time I set aside just for you and makes sure I'm not turning away another family who could have used that spot. That said, I understand life doesn't always go as planned: partial refunds may be considered under extenuating circumstances, at my discretion.

• Begin implementing strategies from your transition and adjustment plan, whether that means preparing for the upcoming change or supporting your family member's early adjustment, and track your progress.

7. CONFIDENTIALITY

• I'll keep everything you share strictly confidential and won't disclose your name or details without your written consent, except as required by law or to prevent harm.

8. DISCLAIMER

• Coaching isn't therapy and doesn't replace medical, mental health, or legal advice. You're solely responsible for the decisions, actions, and outcomes that come from our work together.

9. LIMITATION OF LIABILITY

• My total liability is limited to the amount you've paid under this agreement. I can't promise specific outcomes, since every family's situation is different, but I'm fully committed to showing up for you throughout our work together.

10. TERMINATION

• Either of us can end this agreement with 14 days' written notice. Payments already made remain non-refundable.

11. GOVERNING LAW

• This agreement is governed by the laws of the State of Florida.
