COACHING AGREEMENT

• Coach will provide, and Client will pay for, dementia caregiver coaching services in accordance with the terms below.

• Onboarding through the AttractWell App.
• One 60-minute + one 30-minute virtual coaching session via Zoom over 2 weeks.
• Recordings of both sessions.
• A comprehensive self-care plan covering: stress assessment, self-care routines, boundary-setting, time management, support systems, and emotional resilience.
• A comprehensive care plan for your loved one covering: daily routines, person-centered care, behavioral strategies, communication, safety, and independence.
• Unlimited email access to Coach for 4 weeks following your second session.
• Lifetime 25% discount on all future programs and offerings.
• Lifetime access to the Resource Vault in the AttractWell App.

• Sessions will be scheduled at mutually agreeable times.
• Coach will make reasonable efforts to schedule within 3 days of completion of pre-coaching homework.
• Either party must provide 24 hours' notice to cancel or reschedule a session.
• Sessions cancelled with less than 24 hours' notice are non-refundable.
• If Coach cancels and cannot reschedule within 5 days, Client is entitled to a refund of that session's fee.

• Total fee: $299, paid in full at time of booking.
• Coaching does not begin until payment is received.
• Additional sessions available at $120/hour (in 30-minute intervals).

• All payments are non-refundable. Payment reserves Coach's time and prevents others from booking that spot. Partial refunds may be granted under extenuating circumstances at Coach's sole discretion.

• Coach will keep all information you share strictly confidential and will not disclose your name or details without your written consent, except as required by law or to prevent harm.

• Coaching is not therapy and does not substitute for medical, mental health, or legal advice. Client is solely responsible for all decisions, actions, and outcomes arising from coaching.

• Coach's total liability is limited to the total amount paid under this Agreement. Coach makes no guarantees regarding specific outcomes.

• Either party may terminate this Agreement with 14 days' written notice. All payments remain non-refundable upon termination.

• This Agreement is governed by the laws of the State of Florida.
